I am trying to understand how the forward proxy handles user authentication/identification when SSL is set to bypass when there is no agent running on the endpoint.

If there is no agent on the machine, the machines traffic will then be identified in the Netskope UI by its source ip. I assume you are referring to a machine/server using IPSec or GRE method.
